Trying to finish my reply to your post. 
I agree that given same sensory input the
corresponding cortex activity will be different for
each person. Therefore the mentioned cortex dictionary
will be different for each person - just like we need
different dictionaries for each nation. That also
implies that mind uploading using the discussed
approach will have to be accomplished from within with
a great effort on the part of the individual.
As for your concern how the cortex activity can be
observed - we know that all neuron and synaptic
activities are based on electro chemical processes. I
think that once there is electric activity even if
very minute - it can be intercepted with sensitive
enough electrodes.
